The Service
Youth‑to‑Adult (YTA) Transitional Support is a short‑term, high‑impact service designed to make the transition into adult services smoother, safer, and more predictable. This service bridges the gap between school‑based supports and adult programs by gathering essential information, reviewing documents, coordinating with sending and receiving providers, and creating a clear, trauma‑informed transition roadmap.
Over 4–6 weeks, we identify strengths, motivators, stressors, communication needs, environmental supports, and safety considerations — all distilled into a practical, easy‑to‑use plan for the next team. Families and providers receive training, simple data tools, and two follow‑up calls to support consistency during the transition period.
This service is ideal for families and teams who want a structured, supportive, and behavior‑analytic approach to one of the most important transitions in a young adult’s life.
