Youth‑to‑Adult Transitional Behavior Support is designed for teens and young adults who are preparing to shift from school‑based or pediatric systems into adult programs, placements, or supports. This service provides a structured, time‑limited process that helps families and teams understand what the transition requires, what behavior‑related needs must be addressed, and how to set the young person up for success in their next environment.
This service includes:

a focused intake and meet‑and‑greet

review of existing behavior plans, reports, and transition documents

plain‑language translation of what those documents mean for adult services

a short‑term Behavior Support Plan or Transition Roadmap tailored to the new setting

training or orientation for caregivers or staff who will support the young person
